When Is Sweating Considered Excessive?

Hyperhidrosis is more than simply sweating heavily in hot weather or during exercise. Excessive sweating may occur even when the body does not need additional cooling.

### Primary Localized Hyperhidrosis

Primary hyperhidrosis commonly affects localized areas such as the underarms, palms, soles or face. It often affects both sides of the body and can significantly interfere with everyday activities.

### When Should Another Cause Be Considered?

Further medical assessment is particularly important when sweating:

- Started recently
- Is generalized across much of the body
- Has changed significantly from the previous pattern
- Occurs unusually during sleep
- Is associated with other symptoms or a new medication

In these situations, the priority is to assess for a possible secondary cause rather than treating the sweating alone.

BOTOX® for Underarm Sweating

BOTOX® (onabotulinumtoxinA) can be used for appropriately selected patients with severe primary underarm hyperhidrosis when topical treatment has not provided enough control.

It works by temporarily reducing the nerve signals that activate sweat glands in the treated skin.

Who May Benefit From Underarm BOTOX®?

  • Underarm sweating is excessive and recurrent
  • Topical antiperspirants have not provided enough control
  • Sweating interferes with clothing, work or everyday comfort
  • The pattern is consistent with primary localized underarm hyperhidrosis

When Is Treatment Postponed or Reconsidered?

  • Active infection or significant inflammation at the injection site
  • Known hypersensitivity to the selected botulinum toxin product or its components
  • A new, generalized or otherwise atypical sweating pattern that requires medical assessment first
  • A medical history or medication profile requiring additional assessment before botulinum toxin treatment

What Does Treatment Involve?

A series of small injections is placed across the affected underarm skin. Treatment planning is based on the area of excessive sweating rather than a cosmetic facial injection pattern.

Sweating commonly begins to decrease within several days, with the effect becoming clearer over approximately 1–2 weeks.

The effect is temporary. Repeat treatment can be considered when excessive sweating gradually returns.

What to Expect After Treatment

Possible after-effects include:

- Mild redness
- Small injection-site bumps
- Tenderness or brief stinging
- Minor bruising

Most patients can return to normal daily activities shortly afterward.

Treatment Options May Include

Areas we assess: underarms, palms, soles, face, and other areas of localized excessive sweating.

  • Prescription antiperspirants
  • Medical treatment when appropriate
  • BOTOX® / botulinum toxin injections for selected localized areas

Botulinum toxin may also be considered for other localized areas in selected patients, including the palms or soles, but the injection technique, comfort measures, functional considerations and expected response differ from underarm treatment. For this reason, treatment of the hands or feet is assessed separately rather than presented as the same protocol used for the underarms.

How quickly does underarm BOTOX® start working?

Sweating usually begins to decrease within several days, with the result becoming clearer over approximately 1–2 weeks.

How long does the effect last?

The duration varies between patients. The effect is temporary, and repeat treatment can be considered when excessive sweating gradually returns. The aim is to reduce excessive sweating to a manageable level — normal perspiration elsewhere in the body continues, and the goal is not to eliminate every amount of sweating.

Is the treatment painful?

The session involves multiple small injections, which can cause brief pinching or stinging. Comfort measures can be discussed when needed.

Do I need laboratory tests?

Not routinely for a typical pattern of primary localized hyperhidrosis. If the sweating is new, generalized or accompanied by other concerning features, selected testing may be recommended through an external laboratory.

Can BOTOX® be used for the palms or soles?

It may be considered in selected patients, but treatment is more individualized and differs from underarm treatment.

What if my sweating is new or generalized?

New, generalized or unexplained sweating should be assessed for a possible medical or medication-related cause before focusing only on reducing the sweating.
